The Active Southern West Virginia Workplace Wellness Award recognizes and celebrates employers that promote employee wellness and create a healthy workplace through policy, systems, and environmental changes.
Employers may qualify for three different levels of recognition – Bronze, Silver, Gold – based on the level of support offered at their workplaces. The Award Distinction is an annual recognition.
This Award Program is proudly brought to you by Active Southern West Virginia in partnership with the West Virginia Division of Health Promotion and Chronic Disease (HPCD).
You may apply for the Workplace Wellness Award if you are a member of Active SWV or have a Work@HealthⓇ certified employee.
Application submissions will be accepted from April 15, 2022 through May 25, 2022.
Qualifying applicants will receive their award decisions by email after the review process has been completed.
Award recipients will be invited to attend a formal award ceremony during the Annual Active SWV Workplace Wellness Conference, which will take place on June 23, 2022 at Stonewall Resort.

In order to receive a Bronze, Silver, or Gold Award, workplaces must meet a set number of Policy, Systems, and Environmental benchmarks in each level. The table below summarizes the different qualification requirements for each award level. Please refer to this table as you fill out your application.
