Put Your Plan in Motion

A wellness plan is only as strong as its execution.

The Act phase is all about implementation. Whether you’re updating policies, changing your physical environment, or engaging staff through wellness programs, this step transforms your planning into real results.

  • Strategic goals from your workplace wellness plan
  • Guidance and resources from Active SWV

Apply for a Capacity Building Grant

Capacity Building grants from Active SWV are awarded to worksites that have at least one Work@Health trained employee. These $1500 grants are intended to provide funds to assist worksites with implementing Policy, System, and Environmental changes as a part of their workplace wellness program. 

Applications will be accepted on a rolling basis that will be reviewed as they are received. Typically, grant applications open November first and close December 10th.  Only one application per workplace will be considered. Grants are competitive. Each application will be objectively ranked by a selection committee using a scoring rubric.

Things to Know:

FUNDING GUIDELINES:

  • Limit all incentives to $25 or less per item
  • No more than 50% of the budget can be for incentives
  • No more than 20% of the budget can be for purchasing food
  • You must have at least 1 employee who is Work@Health® certified
